Entry form

Entry forms must be made on the official form that is enclosed at the supplementary regulations.

The entry form must be filled in properly!!! If not the entry form cannot be accepted.

Entry forms must be delivered to the organizer on May, 29th 2010 at the latest. This date 2010 is deadline!

The organizer confirms rider´s entry forms that are delivered on time and that they include everything that is necessary for registration.

Entry forms that are delivered after the deadline WILL NOT BE ACCEPTED.

The organizer requires riders to use a registration form to the eventual reservation ofaccommodation during the sport event. The phone number : +420 777 008 121, kemphorice (at) seznam.cz .

The organizer doesn´t reimburse the accommodation.
